That puts the typical smoothie below cereal (+38.3 mg/dL), oatmeal (+38.9 mg/dL), and pancakes (+39.9 mg/dL) but above many savory protein options. The dominant lever isn't blending — it's composition: a fruit-only smoothie averages +32.1 mg/dL while a protein-based build averages +26.4 mg/dL, and a high-sugar (≥20g) build runs 54.5% above the low-sugar baseline. Pair the right form with breakfast eggs and the cohort essentially flatlines the spike.

Smoothies sit in an interesting middle of the glycemic landscape — a published GI of 40 and GL of 12 hide a build-driven range that runs from +22.8 mg/dL (chocolate / cocoa) to +33.9 mg/dL (açaí bowl) before any pairing is added. The single biggest lever in the full cohort is total meal carb load: meals with ≥60g of carbs ran 57.3% higher than low-carb logs (p<0.001), and the very-heavy bucket (110g+) reached +47.3 mg/dL — 107% above the light-carb mean of +22.9 mg/dL. Sugar load tracks closely behind, with high-sugar (≥20g) builds peaking 54.5% higher than low-sugar logs. Protein pairing pulls hard in the opposite direction, cutting the response by 24.6% across 35,976 protein-paired meals — the largest mitigating factor measured. Smoothies aren't inherently good or bad for glucose; they amplify whatever you choose to put in them.
This page draws on 44,413 smoothie logs from 7,288 unique Signos members, collected through the Signos production CGM platform. Data were filtered to postprandial glucose responses (PPGR) between 0 and 100 mg/dL to exclude sensor artifacts and implausible readings. The form slot reflects a sub-cohort of 11,883 logs where the smoothie was the primary item; absolute means in the form slot use that as the comparison baseline. Modifier effects were estimated using Welch's t-test on matched pairs against a defined baseline; all reported effects carry p<0.001 unless noted. We report mean glucose peak across a right-skewed distribution — the median single-item spike of +20 mg/dL is meaningfully lower than the mean of +25.8 mg/dL, which is typical for postprandial data. The food was identified in member logs using a regex matching "smoothie" or "shake" as a primary ingredient, excluding milkshakes and protein shakes (which have their own atlas page). Any carb-pair sub-cohort requires at least 100 matched meals before it appears in the recipe builder.
