How to Get Better Results on GLP-1s: 7 Dietitian-Backed Strategies

GLP-1 medications can be genuinely life-changing. For many people, starting one feels like the first time their biology has finally started working in their favor. The food noise quiets. The scale moves. Something clicks that never has before.

But here's what most people aren't told at the start: the medication changes how much you want to eat. It doesn't change what your body needs. And that gap, between eating less and being nourished, is where a lot of GLP-1 results quietly fall apart.

A 2026 systematic review in the BMJ found that people regained an average of nearly 10 kilograms (22 lbs) within the first year of stopping semaglutide or tirzepatide, projecting a return to baseline weight within 12-18 months.¹ 

That's not an argument against GLP-1s. It's an argument for using them strategically and building the metabolic foundation that makes results last beyond the prescription.

Here are 7 evidence-based ways to do exactly that.

1. Eat More Protein to Prevent Muscle Loss on GLP-1s

This is the most important nutritional shift you can make on a GLP-1, and the one most people miss.

When appetite drops significantly, protein intake tends to drop with it, often without you realizing it. And when the body is losing weight rapidly without adequate protein, it doesn't just lose fat. It loses muscle too. A 2024 network meta-analysis found that semaglutide and tirzepatide at their highest doses were among the least effective GLP-1 medications for preserving lean mass, with lean tissue accounting for 26 to 40% of total weight lost across multiple trials.² A 2025 prospective study (the SEMALEAN study, 115 patients over 12 months) confirmed meaningful lean mass losses in the first seven months of treatment.³

This matters because muscle is metabolically active tissue. It drives insulin sensitivity (how well your cells can clear glucose from your bloodstream), resting metabolic rate, and your body's ability to maintain weight loss over time. Losing it while on a GLP-1 quietly undermines the results you're working toward.

The good news: protein intake makes a meaningful difference. Research presented at ENDO 2025 by scientists at Massachusetts General Hospital and Harvard Medical School found that higher protein intake during semaglutide treatment was associated with significantly better lean mass preservation, particularly in women and older adults.⁴ A 2025 case series found that patients who prioritized resistance training and consumed higher protein intakes either preserved or actually increased lean soft tissue during GLP-1 treatment.⁵

What to do: 

  • Aim for 25 to 35g of protein per meal, distributed across the day rather than loaded into one sitting, and eat it first before carbohydrates or fat
  • Prioritize easy-to-digest sources when appetite is low: eggs, Greek yogurt, cottage cheese, fish, and chicken. If food feels unappealing, a high-quality protein shake counts.
  • Track body composition rather than scale weight alone. Muscle loss can be invisible on the scale while quietly impacting your metabolism and long-term results.

2. Meet Your Nutrient Needs Even When Your Appetite Is Low

Reduced hunger does not mean reduced nutritional need. Your body still requires adequate calories and micronutrients to function well. When those needs aren't consistently met, the downstream effects are predictable: fatigue, dizziness, hair thinning, muscle loss, and metabolic adaptation (where the body slows its metabolism in response to chronic under-fueling). 

Nutrient deficiencies are particularly worth watching as total food intake drops. Iron, magnesium, B12, omega-3 fatty acids, and potassium are commonly under-consumed when people are eating significantly less, and each plays a meaningful role in energy, metabolic function, and recovery.

What to do: 

  • Focus on nutrient density over caloric restriction. Prioritize whole, minimally processed foods that deliver more per bite: quality proteins, leafy greens, healthy fats, legumes, and colorful vegetables.

    • B12: Eggs, salmon, sardines, beef, Greek yogurt, cottage cheese
    • Iron: Lean red meat, lentils, spinach, pumpkin seeds, fortified cereals (pair plant-based iron sources with vitamin C to improve absorption)
    • Vitamin D: Fatty fish like salmon and mackerel, egg yolks, fortified dairy or plant milks (most people need a supplement in lower-sunlight months)
    • Calcium: Greek yogurt, kefir, sardines with bones, leafy greens like kale and bok choy, fortified plant milks
    • Magnesium: Pumpkin seeds, almonds, dark leafy greens, black beans, dark chocolate
    • Potassium: Avocado, salmon, white beans, spinach, sweet potatoes, bananas
    • Omega-3s: Fatty fish like salmon, sardines, and mackerel; walnuts; flaxseeds; chia seeds
    • Zinc: Beef, oysters, pumpkin seeds, lentils, cashews, eggs
  • Avoid going more than 5 to 6 hours without eating, and aim for consistent meal timing day to day. Long gaps and irregular timing can trigger glucose instability and rebound hunger that works against the medication's benefits.

3. Stabilize Your Blood Sugar While Taking a GLP-1

A common misconception: if appetite is controlled, blood sugar must be fine too.

Not necessarily. GLP-1 medications improve post-meal glucose regulation on average, but individual responses vary considerably based on what you eat and when. People who go long stretches between meals and break that fast with carbohydrate-heavy foods can still experience meaningful glucose spikes followed by energy crashes and cravings that feel completely at odds with being on an appetite-suppressing medication.

Glucose stability matters because unstable blood sugar, even at lower caloric intakes, still drives the spike-and-crash cycle that produces fatigue, food cravings, and afternoon energy dips. 

What to do: 

  • Pair carbohydrates with protein, fat, and fiber at every meal. Choose slower-digesting carbohydrates like sweet potatoes, legumes, oats, and whole grains over refined or liquid sources, which spike glucose faster than almost any solid food. 
  • Build consistent meal timing day to day, as irregular eating patterns can drive glucose variability even when individual meals are well-composed. 
  • Walk for 10 minutes after meals: a 2025 study in Scientific Reports found that a 10-minute walk immediately after eating reduced post-meal glucose peaks significantly compared to remaining seated, with timing (right after eating) being a key factor.⁶ Seated calf raises have also been shown to improve glucose disposal while sitting.

4. Strength Train to Preserve Muscle During GLP-1 Weight Loss

The scale tells you how much you're losing. It doesn't tell you what you're losing.

This distinction matters more on GLP-1s than almost any other intervention. Rapid weight loss combined with reduced calorie intake creates conditions where the body breaks down muscle alongside fat for energy. As covered above, the data on lean mass loss during GLP-1 treatment is significant enough to take seriously.

Resistance training is the most evidence-backed intervention for preserving lean mass during weight loss. A 2025 review in Frontiers in Clinical Diabetes and Healthcare confirmed that resistance training is among the most effective strategies for mitigating lean mass loss during GLP-1 therapy.⁷ Separate research has shown that every 10% increase in muscle mass relative to body size is associated with an 11% reduction in insulin resistance, making muscle preservation a genuinely metabolic goal, not just an aesthetic one.

What to do: 

  • Incorporate resistance training 2-3x/week. It doesn't require a gym: bodyweight exercises, resistance bands, and loaded carries all count. Consistency matters more than intensity. Pair sessions with 25 to 35g of protein within two hours post-workout.
  • Beyond structured exercise, keeping daily movement high matters too: frequent walking, taking the stairs, and avoiding long sedentary stretches all support insulin sensitivity and help preserve the metabolic activity that rapid weight loss can erode. 
  • If the scale is moving quickly after the first couple of weeks, that's worth discussing with your clinician. Faster is not always better when lean mass is on the line.

5. Support Gut Health to Reduce Side Effects

GLP-1 medications slow gastric emptying by design. This is largely why they produce such powerful satiety. It's also why digestive side effects are the most commonly reported experience on GLP-1s: nausea, bloating, constipation, and a persistent sense of heaviness after eating.

Supporting your gut proactively makes the experience more manageable and supports the metabolic benefits of the medication at the same time. Your gut microbiome produces short-chain fatty acids that help regulate insulin sensitivity, blood sugar, and appetite signaling. A disrupted gut doesn't just cause discomfort. It can quietly worsen the metabolic environment you're trying to improve.

What to do: 

  • Eat smaller, more frequent meals rather than 2 or 3 large ones, and eat slowly, stopping before you feel full (fullness signals are delayed with slower gastric emptying, so you'll often feel overly full 15 to 20 minutes after the fact). 
  • Avoid large or high-fat meals if nausea is an issue, and avoid lying down after eating. 
  • Increase fiber gradually rather than aggressively, and prioritize soluble fiber sources like oats, chia seeds, and cooked legumes if constipation is a recurring issue. 
  • To support your microbiome, eat a diverse range of plant foods weekly and include polyphenol-rich foods like berries, olive oil, and dark cacao alongside resistant starches like cooked and cooled potatoes, green banana, and legumes. 
  • Introduce fermented foods like kefir, kimchi, and yogurt gradually if tolerated. 
  • Ginger tea or peppermint can help manage nausea in the early weeks if needed.

6. Hydration and Electrolytes Matter More Than You Think

When you eat less, you drink less too. A significant portion of daily fluid intake comes from food itself, and most people don't consciously compensate when their food volume drops.

The result is a pattern many GLP-1 users experience without connecting it to hydration: persistent fatigue, headaches, muscle cramps, dizziness, and worsening constipation. These symptoms are often attributed to the medication when they may actually be dehydration or an electrolyte imbalance underneath the surface.

Sodium, potassium, and magnesium are worth paying particular attention to when food intake drops. Magnesium deficiency in particular has been associated with increased insulin resistance and worsened metabolic outcomes. Each of these electrolytes plays a role in energy production, muscle function, digestion, and glucose regulation.

What to do: 

  • Start every morning with a full glass of water before coffee. Drink consistently throughout the day rather than in large quantities at once, as large amounts of fluid at one time can worsen nausea in people with delayed gastric emptying.
  • If you're experiencing persistent fatigue, muscle cramps, headaches, or dizziness not fully explained by the medication, add an electrolyte supplement without added sugar and see if it makes a difference. Sodium, potassium, and magnesium are the 3 most worth prioritizing. 

7. Use the Low Appetite Window to Build Habits That Make Results Last

This is where everything else comes together.

The appetite suppression a GLP-1 creates is genuinely valuable. But its deepest value isn't just in helping you eat less. It's in giving you the space to rebuild your relationship with food, hunger, and your own metabolic patterns, without the constant pull of cravings getting in the way.

The most successful GLP-1 users don't simply let the medication do the work. They use it to get curious: what does real hunger feel like vs. habitual hunger? Which foods leave me energized vs. crashing? What does my body actually need vs. what I was eating out of stress or habit?

What to do: 

  • Use the lower-noise period to build repeatable habits around protein intake, meal structure, movement, and sleep; these are the habits that will carry your results forward.
  • Prioritize sleep consistency: even modest sleep deficits increase insulin resistance and disrupt the hunger hormones that GLP-1s are working to regulate. 
  • Manage stress actively, as cortisol raises glucose independently of food intake and can work directly against the medication's benefits. 
  • Start thinking about maintenance before you stop the medication, not after. Work with a clinician or registered dietitian on dose titration and side effect management throughout treatment, not just at the start. 
  • Resist the temptation to rely on medication as the full strategy. The medication quiets the noise. What you build in that quiet is what determines what lasts.
